Our partnership with SilverSummit Healthplan emphasizes equitable access to care and the reduction of health disparities across Southern Nevada. Through shared efforts in outreach, preventive screenings, and chronic care management, we connect more members to consistent primary care and vital support services. Together, we are fostering healthier neighborhoods and improving the quality of life for our most vulnerable residents.
Molina Healthcare
Our partnership with Molina Healthcare is focused on increasing the number of assigned patients who establish care with our organization. As one of the four Managed Care Organizations in Nevada, Molina provides Medicaid benefits and works to reduce gaps in care for vulnerable populations. Through our collaboration, we are able to leverage Molina’s extensive network and resources to connect with individuals who may otherwise face barriers to accessing healthcare services. By working together, we are able to improve health outcomes for our patients and promote equitable access to care for all members of our community.

Our partnership with CareSource aims to advance whole-person health through expanded access to quality care and community-based support. With CareSource’s strong focus on innovation and social determinants of health, our collaboration helps connect patients to preventive services. Together, we are building healthier communities and creating sustainable pathways to wellness for Nevada’s Medicaid members.

Our partnership with Chicanos Por La Causa (CPLC) supports individuals and families through coordinated healthcare services. By bringing on-site medical care to CPLC program participants, we address both physical and social determinants of health. We are both empowering individuals to build healthier lives and strengthening the well-being of our community.
Dolores Huerta Resource Center
The Dolores Huerta Resource Center broadens our reach within North Las Vegas by offering a bilingual, community-driven environment where residents can access healthcare and support services. This collaboration enables us to meet individuals where they are, ensuring language, culture, or circumstance never stand in the way of quality care and connection.
Las Vegas Rescue Mission
Our partnership with the Las Vegas Rescue Mission brings on-site medical care directly to individuals experiencing homelessness. Through regular outreach clinics, we provide primary health services in coordination with the Las Vegas Rescue Mission. Together, we are addressing the immediate healthcare needs of individuals in crisis while supporting their journey toward stability, healing, and renewed hope.
Abundant Life Church, Amargosa
Abundant Life Church in Amargosa Valley provides a vital community anchor for our Rural health services. By hosting healthcare services within their trusted local space, residents can access care closer to home without transportation barriers. In partnership, we are strengthening rural health access and creating lasting connections built on trust and compassion.
The Vine Community Church, Pahrump
The Vine Community Church in Pahrump serves as a cornerstone in extending healthcare access to Nye County residents. By offering space and community outreach support, The Vine Community Church helps us connect with families who otherwise face limited access to services. In shared service, we are improving rural health outcomes through collaboration rooted in faith and community care.
La Quinta
La Quinta provides a consistent outreach site for our Medical Outreach Clinic, offering a safe and familiar environment for those experiencing homelessness or housing insecurity. Together, we meet patients where they are, delivering compassionate care, behavioral health support, and connections to resources that help individuals take their next steps toward stability.
Rodeway Inn
Through Rodeway Inn, we bring essential health care directly to individuals and families living in transitional or unstable housing. Collectively, we are providing compassionate, accessible care that helps restore health, rebuild confidence, and strengthen pathways toward independence.
Galaxy
The Galaxy partnership allows us to bring essential healthcare services directly to individuals and families in need. Through our Mobile Outreach Clinic, we provide on-site primary care, behavioral health support, and connections to vital community resources. By meeting patients where they are, we help bridge critical gaps in care and support their journey toward improved health, stability, and self-sufficiency.
Casa de Luz
Casa de Luz strengthens our outreach mission by offering a community-centered space where individuals and families can receive healthcare and wellness support. Through mobile clinics, we extend care beyond clinic walls, nurturing both physical health and community connection throughout Las Vegas.
Catholic Charities
Our partnership with Catholic Charities of Southern Nevada allows us to integrate healthcare into a continuum of social support for those experiencing homelessness, poverty, migrant, and/or refugee situations. Together, we deliver coordinated services that address both medical needs and the root causes of health inequities, ensuring every individual receives care with dignity and respect.
Catholic Charities: St Vencent Apartments
At the St. Vincent Campus, our collaboration with Catholic Charities allows us to deliver primary and behavioral health services to low-income, elderly, and residents with disabilities. Hand in hand, we are reducing barriers to care and providing a healing space that prioritizes compassion, respect, and recovery.
Foster Kinship
Foster Kinship supports families caring for children in relative or kinship placements by ensuring access to essential health screenings and exams. Our partnership helps strengthen family stability and child wellbeing through compassionate, coordinated care and community connection.
Guatemalan Consulate
The Consulate primary goal of course is to help the Guatemalan community to be able to process their needed documents, but their desire is to be a place where the Hispanic Community can access a number of wrap around services. Including but limited to, medical, legal, education, etc.
Lutheran Social Services
Lutheran Social Services provides vital support to individuals and families in need by offering daily hot meals, food box distributions, and other wraparound services that promote stability and wellbeing. The organization serves as a safe, welcoming place for individuals experiencing homelessness to find relief from the elements and access essential resources. Together, we share a commitment to meeting basic needs and creating pathways to health and stability for our community.
Outreach Pop-up Clinic Partners
The Lovewell Center
The Lovewell Center operates as a membership-based program where individuals experiencing homelessness can connect, belong, and rebuild stability. Members earn points through participation in classes, chores, and other on-site programs, which can be used for essential services. Through our partnership, we provide on-site medical care and resource navigation in a compassionate, community-centered environment. This collaboration helps members maintain their health and wellbeing as they work toward personal growth, recovery, and renewed independence.
Safe House
Through our partnership with The Safe House, we provide trauma-informed care for survivors of domestic violence. Our collaborative approach ensures that individuals and families receive confidential medical services, behavioral health support, and pathways to long-term healing and empowerment.
Shade Tree
The Shade Tree provides a safe haven for women and children experiencing crisis and homelessness—offering on-site healthcare, behavioral health support, and essential resources. Uniquely, The Shade Tree also allows residents to bring their pets, removing a significant barrier for many survivors who might otherwise remain in unsafe situations. Together, we are helping families find safety, healing, and stability through compassionate, holistic care.
Freedom House
Freedom House supports individuals transitioning out of homelessness, recovery programs, and incarceration by integrating comprehensive health and wellness services into their path toward independence. Through collaborative outreach and on-site clinical care, we help residents restore their physical and mental health, rebuild stability, and reclaim their sense of purpose as they reintegrate into the community.
Salvation Army
Our partnership with The Salvation Army focuses on their Adult Rehabilitation Center, a residential recovery program supporting individuals on their journey to sobriety. Participants live on-site as they work toward long-term recovery, and our team provides medical and behavioral health services to help them maintain their health throughout the process. Together, we are supporting physical wellness, mental stability, and sustained recovery for individuals rebuilding their lives.
Governmental/City Partners
City of Las Vegas
Our partnership with the City of Las Vegas consists of providing services at the Recuperative Care Center. Through this collaboration, we are able to offer critical medical care and support to individuals experiencing homelessness while recovering from an acute illness/injury where the lack of acuity no longer justifies continued hospitalization. Together with the City of Las Vegas, our dedicated healthcare professionals work tirelessly to ensure that every individual is treated with compassion and given the care they need to regain their health and wellbeing.

Our partnership with the Nevada Primary Care Association is focused on advocating for the healthcare needs of Nevadans at the city, state, and federal level. Together, we work to ensure that individuals and families have access to high-quality, affordable healthcare services, regardless of their income or background. Our collaborative efforts involve engaging with policymakers, stakeholders, and community members to promote policies that support equitable healthcare access and address the unique needs of our state’s diverse populations.
