At Hope Christian Health Center, we understand that managing chronic diseases or navigating complex medical and/or social situations can be overwhelming. That’s why we offer a Care Management program to provide extra assistance and support. Our care managers work directly with our providers and engage care team meetings resulting in the integration of total patient care.

Hypertension Management Program

We are proud to offer a hypertension management program to qualifying patients in order to help each enrollee maintain healthy blood pressure. Our providers make the referrals and our care manager works with each patient to ensure successful outcomes. Participants use Welch Allyn blood pressure cuffs and take self-measurements at home. Readings are entered into the Welch Allyn patient portal where both our care manager and the corresponding provider review regularly. 

Our care manager provides training and support to patients during the one-year program along with several check-ins. If you are interested in learning more or participating, please let your provider know!
