Patient-Centered Care is Our Priority: Our Model Focuses on the Needs of Patients
At Hope Christian Health Center, we are dedicated to providing patient-centered care through the Medical Home model. This means that we prioritize the needs of our patients and work to coordinate all aspects of their care, including lab and imaging records, specialty care consults, urgent care, and hospital records. By maintaining a comprehensive view of each patient’s health, we are able to make informed decisions about their care.
Patient-Centered Medical Home Explained
Hope Christian Health Center is pursuing recognition as a Patient-Centered Medical Home (PCMH) from the National Committee for Quality Assurance (NCQA). The NCQA is a non-profit organization that evaluates and reports on the quality of various healthcare programs, including managed care plans, physician organizations, and disease management programs. The PCMH program was designed to recognize clinician practices that provide systematic, patient-centered, coordinated care and encourage patient involvement and communication.
