HCHC is proud to offer accessible, quality care directly in the community. This is accomplished through the partnerships and locations listed below. All primary care services, labs, and screenings are offered at each outreach site. Be sure to let your family, friends, and community know about the outreaches occurring near them so they can establish medical care!

HCHC partners with Chicanos por la Causa to provide primary care services every Tuesday at the Courtyard Homeless Resources Center.
