Our Medical Outreach Clinic (MOC) is Hope Christian Health Center’s first mobile unit! We are excited to increase access to healthcare for our community by coming directly to them. The full scope of primary care and lab services offered at our clinics will be available to MOC patients along with specialty referrals. Our MOC team consists of one provider, one medical assistant, a community health worker, and a patient navigator. We look forward to helping to improve the health outcomes of individuals in our communities.
